This website provides information about HealthPlex’s allied health, workplace rehabilitation and injury management services for referrers, employers, insurers, patients and other visitors. It is intended for general informational purposes and to facilitate enquiries and referrals — it does not itself provide clinical services.

3. No patient-practitioner relationship

Browsing this website, submitting an enquiry, or downloading information from it does not, on its own, create a patient-practitioner relationship with HealthPlex or any of our practitioners. A clinical relationship begins only once we have accepted a referral or booking and you have attended, or received care through, an appropriate consultation.

4. Enquiries, referrals and bookings

Where this website allows you to submit an enquiry, referral or booking request, you agree to provide accurate and complete information. Submitting a form does not guarantee acceptance of a referral, availability of an appointment, or eligibility for a particular funding pathway (including Medicare, NDIS, DVA or WorkCover) — all such matters remain subject to our clinical and administrative processes.

5. Licence to use this website

Subject to these Terms, we grant you a non-exclusive, royalty-free, revocable, worldwide and non-transferable licence to access and use this website for your own personal purposes connected with HealthPlex’s services. All other uses are prohibited unless we give you prior written consent.
